mine was chinese too.he has all the forms at his clinic.i didnt took any form with me This is family case so doesnt make any difference if we will use uci on requested paper.
I did mine the way I told above.so jz shared my experience.

Hey hunny007.. I called the IMO office(Where i got my medicals done) and asked them about the sputuum result and whether the results have been mailed to new delhi or not. So you can call and ask them too..

Well, scan/email form or fax and get it back signed
I took a digital signature for my folks on my Note 2 Android phone, saved as .jpg and then inserted via PDF - so I reuse that all the time, but you need it first time

Doctor said me that its just ur father's name that is coming on the system with that uci.. U should have urs separatey...until then they cant do anything... I dont know what to do now.... We talked to our lawyer he says there shouldnt be any problem...

Did you search your student permit? also, as per khaira, UCI should be same for you to - try different DMP or tell him to do under yours
I know Khaira said that his DMP got it done and he got it passed but if you read the meds form, it says that if you are outside India, you need to request ND med forms which is what we got

DMP called last week my mother in law (alone PA) for consultation report regarding Diabetes and possible kidney problem (appointment was on June 12, 2013)
What does it mean?
Main Medicals were done on March 17, 2013 and there was no further tests requested.
CHC NEW DELHI asked RPRF on April 26, 2013

If ecas didn't show medicals received means it was being worked on. Did you get furtherance request from ND?

Well, ND is supposed to ask you - could be delayed due to strike and they are pretty slow - ( user: sakamath might be able to comment on this )
Ask DMP if ND requested and also order GCMS Notes ( see my sig ) to see File status
