Since everyone is talking about medicals I have questions. Please reply from your experience.

If someone has any medical issue, say heart disease ,Does the panel physician orders more tests before sending the medical to RMO ?Or panel physicians just mention and RMO requests later for furtherence? What was your experience?

If someone has that sputum test are the results in one week or 8 weeks? How long doesit take?
Thanks

Hello Guys, ( especially Rajaji ), so you probably know my case quite well now

I got in touch with MP who has sent fax to ND, asking to make sure email is correct, resend the docs and make sure file is not closed
Now, I was reading about Medicals and got lilttle lost if the UPFONT MEDS will be an issue? and there are changes to this so I found this

http://tomcic.wordpress.com/tag/imm-1017/

New immigration medical examination process in effect as of November 1st

The forms, listed below, are available on the CIC website as well as under the “Forms” section on the CIC intranet site.

IMM 5419: Medical report (Revised)
IMM 5725: Assessment of Activities of Daily Living (New)
IMM 5726: Mini Mental Examination (MME) (New)
IMM 5727: Global Assessment of Functioning Scale (New)
IMM 5728: Acknowledgement of HIV Post-Testing Counselling (New)
IMM 5733: Instructions for Pregnant Client – X-Ray Deferred (New)
IMM 5734: Specialist’s Referral Form (New)
IMM 5738: Chart of Early Childhood Development (CECD) (New)
IMM 5743: Client Consent and Declaration (New)
IMM 5544: Resettlement Needs Assessment Form (Revised) (Available only on Intranet)

In support of the release of the new GCMS 4.0, all previous versions of the IMM 1017 form as well as the Appendix C will no longer be used Instead, two newly-revised IMM 1017 forms have been created:

IMM 1017: Medical Report: Client Biodata and Summary – not available on the CIC website as it will be GCMS-generated along with medical instructions
IMM 1017B: Upfront Medical Report: Client Biodata and Summary – not available on the CIC website as panel physicians will have these forms in their office.

NOTE: With the implementation of the new IMM 1017B, the Appendix C will no longer be required.

====================================

The DMP told me today since I don't have the IME # and due to changes, UPFRONT FORM will be used so the form I have a copy is IMM 1017B

They are waiting for Lab Results and will send it next week

You think this is OK? or should I wait for ND's response to Delhi for paperwork, go back to DMP and fill the regular form which they had with IME #

Also, you said that the additional paperwork from Delhi ( the package which Missed ) has IME#, then how come it wasn't in GCMS notes?

Hello user828: The information you posted above is available on CIC website for a long time and I had posted the link to that earlier also. Seems it did not get attention. Here is the link again:

Operational Bulletin 471 - October 31, 2012
Changes to the Immigration Medical Examination Process
http://www.cic.gc.ca/english/resources/manuals/bulletins/2012/ob471.asp
With the changes to IME process, all ME forms have been changed or revised. Some new forms have been added.

Extract from anticipated Panel Members Handbook

4.3 Medical Report: Client Biodata and Summary (IMM 1017)
CIC has implemented one standard form, the Medical Report: Client Biodata and Summary (IMM 1017), for all immigration categories. This form displays the following:

Client information (biodata);
Immigration information;
IMM Type: EDE; Non EDE; Refugee-Overseas
IME grading; and
Panel Physician Declaration.

The Medical Report (IMM 1017) issued by CIC includes information on the type of immigration category (IMM Type):
Excessive Demand Exempt (EDE);
Non-Excessive Demand Exempt (Non-EDE); and
Refugee-Overseas.

EDE, Non-EDE, Refugee– Overseas

EDE clients are those who are exempted from assessment of excessive demand on the Canadian health care system.

EDE clients are assessed for
Danger to public health
Danger to public safety

Non-EDE clients are assessed for
Danger to public health
Danger to public safety
Excessive demand on the Canadian health care system

Refugee–Overseas clients are also EDE.
NB: Refugee-Overseas – Panel physicians must complete and submit the IMM 5544 Resettlement Needs Assessment Form.
NB: EDE clients include refugees, refugee claimants and certain family classes and individuals with protected person status.

The Medical Report (IMM 1017) form is typically issued by CIC with the client information and immigration information sections completed.

Clients who have been issued a Medical Report (IMM 1017) must present the form to the panel clinic when presenting themselves for their IMEs. The form will include two client identifiers: IME number and UCI number. (NB: This form may not include a client photo).

eMedical-enabled Panel Clinics
 Clinic staff will search for the client health case in eMedical using the IME# (search may also be done using client name and DOB or UCI#)

Clinic staff should confirm the client's identification by comparing the client's information with the valid government-issued photo ID. See section 4.1.2
A live photo of the client is taken and uploaded to eMedical to verify identity throughout the life cycle of the IME.
The IME and all of its components are then completed in the eMedical system.

Paper-based Panel Clinics
Clinic staff should confirm client's identification by comparing the client's information with the valid government-issued photo ID. See section 4.1.2
Clinic staff must attach a client photo (provided by the client) to the Medical Report (IMM 1017).
The Medical Report (IMM 1017) will be completed and submitted along with all other IME forms.

In my Opinion, PP should have used Form 1017 instead of 1017B. Will it make a difference? I do not know the answer to this. As far as ME tests and reports are concerned it will be same for both forms.

You can ask your PP to search for the CLIENT HEALTH CASE in eMedical using client name and DOB or UCI#. Clinic staff may be able to do so if Panel Clinic is eMedical Enabled. If successful IME# will be available in the system.

^^

Concern is I read now that EDE UPFRONT is for certian family class ( such as spouse ) and not for PARENTS, but does this hold true after the new changes
In the from we got today its checked for

Family (EDE)
Familee (EFC)

Also, is IME actually mentioned in the forms from Delhi, I am looking at the template and its nowhere in there ( unless it's next to APPLICATION on the front page ) which I assumed was for File # such as F00000

I am interested to know from those who got the request from Delhi and got their meds done in Canada, what forms did the DMP fill for them

EDE UFM Exams are for spouses, common law partners and their dependent children, I think it is still same even after changes

I think, most applicants fall under NON-EDE. EDE clients include refugees, refugee claimants and certain family classes and individuals with protected person status.

When CIC issues Form 1017 it is pre populated with Client information, IME# and UCI. As I noted in my previous post, if your PP is e-Medical enabled, clinic staff may be able to pull IME# by searching for Client Health Case using client name and DOB or UCI.

I am interested to know from those who got the request from Delhi and got their meds done in Canada, what forms did the DMP fill for them
New instructions are very much clear and require all PP's to use New Forms as well as follow new procedures.

This doctor doesnt have that the tools to grab that but I am still not clear about your last part. Did ND LVO users get 1017 with pre-filled IME# and UCI - as they only get INSTRUCTIONS FOR MEDICAL EXAMINATION -A , it doesn't have IME# anywhere so how did people who have parents visiting Canada get the meds done? without IME#, as the doctor today refused w/o UCI and File # only thus led to this UFM which he assumes is OK for parents but as I research, it's not

I am also waiting for reply from 'glittershine" he got it done as he waited for Delhi to send him OUTSIDE India form but no one replied and he got it done

I also think PP should have used NEW 1017 instead of 1017B.

Hope glittershine reads and replies as that information may answer some questions

Its the new one as far as I can tell

IMM 1017 B (12-2012)

Also, since they are expecting Medicals anyways, what is the difference it goes via A form vs B form as long as the form has UCI and File #? I know this is used for Spouses but can I explain in a letter about the reasoning
1) Delay in getting meds due to wrong email
2) Probable delay in requesting OUTSIDE India Specific form
3) DMP offering UMR considering I wanted to save more delay

Frankly, my heads spinning now

The Medical Report (IMM 1017B UFM) is provided to panel clinics by the Health Branch. These forms are pre-populated with a Unique Medical Identifier (UMI#) and a barcode. Does your copy have a UMI#?

HMB should still be able to identify the ME reports on the basis of UCI. As each Client Case also has IME#, hope that HMB will be able to resolve this correctly (I think it should not be a problem but it is all guess).

You can request your PP to contact HMB/RMO via Fax or Phone and ask for correct procedure.

It gets stressful. I hope that everything works out okay for you. Take Care.

BTW, did you look for IME# in GCMS notes under section MEDICALS?
