As I noted in my previous post, if your PP is e-Medical enabled, clinic staff may be able to pull IME# by searching for Client Health Case using client name and DOB or UCI.Also, is IME actually mentioned in the forms from Delhi, I am looking at the template and its nowhere in there ( unless it's next to APPLICATION on the front page ) which I assumed was for File # such as F00000

The Medical Report (IMM 1017B UFM) is provided to panel clinics by the Health Branch. These forms are pre-populated with a Unique Medical Identifier (UMI#) and a barcode. Does your copy have a UMI#?user828 said:Its the new one as far as I can tell

HMB should still be able to identify the ME reports on the basis of UCI. As each Client Case also has IME#, hope that HMB will be able to resolve this correctly (I think it should not be a problem but it is all guess).Also, since they are expecting Medicals anyways, what is the difference it goes via A form vs B form as long as the form has UCI and File #? I know this is used for Spouses but can I explain in a letter about the reasoning
1) Delay in getting meds due to wrong email
2) Probable delay in requesting OUTSIDE India Specific form
3) DMP offering UMR considering I wanted to save more delay
