Hello all
today I got Medical & Pcc request for my mother, file was transferred to CPP Ottawa on May 2,2012.
do not have any dependant on file.

in a separate email which is addressed to my mother, also received today they have asked
1) Death certificate for my father ( my father expired 10 years back even before i applied for immigration)
2) proof of relationship with sponsor i.e. with me.

other than my old Indian passport I do not have any document that can prove my relationship.
that I already submitted with documents.

please advise what should I do.

Birth certificate, which shows your parents name.

those transfers of file is not strange .It happened to all of us in first quarter of 2010.Do you remember getting back all the dox with a letter that they got the photocopies back then.Files were electronically tranferred then.

Shuga, in your case did it always take at least 30 days? Or were they ever faster than that ? :)

never faster, always 30 days. And the copy of the file was made on the day when CIC received a request, not on the day when they sent it back.

Kind of annoying that they make a copy on the first day :-) I understand why (they need to have it reviewed / approved for release etc.) but still...

One of my friend got medicals for her mother from CPC O, two things they asked for are:

1. Proof of relationship to the sponsor
2. Secondly the death certificate of his father.

Other than old passport he doesn't have any thing in hand. He already submitted the copy of the passport to establish the relationship between him and his mother.

I know that he needs a sworn affidavit from a lawyer to establish his relationship to his mother. eg marriage certificate of his mom and dad.

Please if anyone has the standard language of the sworn affidavit to establish the relationship. Please help me out.

I will be thankful to you all for the great help I am getting from this thread.

my birth certificate has my name that I used never. in India they used to put any name when they register . I can get my birth certificate , but my name may be different.

did you try to use her ID? can you see anything with her ID?
 
Three days ago, I applied for GCMS notes. I just filled the form with file number and UCI number of my mother and my mother's file is transferred to LVO and wrote the LVO name. After reading comments on this thread, I feel that I should have written a letter. Just filling the form and sending my mother's consent form for releasing information along with $5.00 is not sufficient or we have to specify our question. Please give me details, I have never seen the GCMS notes before. I don't have any clue. Reading on this thread only I tried to order the GCMS notes.

Please give me details, if I have done something wrong.

No letter is required. Properly filled form along with consent and fees is sufficient. They do ask for copy of Passport or PR Card, if this is first request.
