Do we need to write(native language) them just for applicant and dependent( Parents name ), or for all columns like children's, sister/brother name?

one more important Question...
I sponsored my parents who are back home. Now my brother( but he didn't sponsor them) filled the forms for our parents who lives two streets down from my place. He took care of it because he was home for a while. I just went thorough them and complete the missing info in the forms.
Now he has paid the fee himself for parents( RPRF). Now question is on the receipt whose name and address we need to put.
1. Prinicpal applicant name and there address back home.
2. Sponsor name and my address.
3. my brothers name and his address( its his credit card)
4. Principal applicant name and Brothers address
5. Principal applicant name and sponsors address(my address)
Please guys which one out of above five because on the receipt it says payers information.
