Posting this question on behalf of my friend who applied for his Mom.

He got his AOR last week. In the letter, he was asked to provide his dad's death certificate and re-submission of Additional Family Information form.

Question is that he has provided filled and signed Additional Family Information form along with his submission. In the letter, the reason for re-submission of this letter is very generic saying it is incomplete. We looked at the copy of submission and see nothing as incomplete. His mother does not have any dependent either.

Has anyone received the re-submission request for this form?

Yes, we looked at the copy of submission and see Names properly written in English and Chinese. And Extended family information is also written. Form is signed and dated as well. We left the sections blank that were not applicable.
 
@YULIA-2016 In the new tracker, my information has been mistaken, Aug 2, 2019 is the date of biometrics request not PCC. Still waiting on PCC request and Medical request.

Check all the date of birth, place of birth and relationship columns. Many people missed some of those and asked to re-submit.

By any chance would there be a mismatch b/w the family members identified (living or deceased) in this form submitted as part of PR application AND a copy of this form previously submitted for a visitor VISA application?

If not, then I suggest you call IRCC for clarification and / or resubmit the form with a note that this form is complete.

We didn't put NA in the blank sections, but I agree a finicky VO may want to see that.
 
I have a question. do we need to submit a separate IMM5562 Travel History for each of our parents? or just for the principal applicant, with the spouse section filled?

Received a request for Signed IMM 5669, parents traveled to US between June and October. Should I update the address details on this form before sending it??
 
Can you please ask @robertp to update the tracker from Kingston LVO to Los Angeles. Cause I was confused there is no LVO in Kingston.
There's definitely an LVO in Kingston. I used to drive past it everyday.

Did they ask about their PGP application # bcoz i am still confused that how they can link medical results with passport. My understanding was that they link to ur file # even if we go for upfront.
They didn't ask for anything else other than the passport, I think the Hospital have access to the CIC system and they can feed the data in it.
 
Anyone from London Visa Office, please share ur PCC request sample if u received.
