Hi everyone, i am filling out my forms to apply to sponsor my parents (we were invited to apply) and i have a question..... on Form IMM 5768 (Financial Evaluation form) where we list our employment history... i am confused about the "gross income and benefits" section. Do i have to go back and find T4's for each year for this? Or will they be able to access this info because i am allowing them access to my CRA file? What does the benefits part mean? Is that a monetary value for extended health benefits? Or does it mean benefits in terms of income? Thanks in advance!


Gross income mentioned in 150 of NOA for 2017, 2016, and 2015 will be something you will put.

For Benefits, I think if you were not employed for certain period and you mention UNEMPLOYED in one of the columns in the Employment History table of IMM 5768, in that case you might have to put the EI amount you received during that time.

thank you! So even though i may have been employed with the company for more than three years, i only put the income from my NOA's for 2015-2017?
 
also, i keep seeing questions about a medical exam... there are no instructions about getting a medical exam on the application package. Is this something that happens further down the road?

Yes, medical checks will happen later after PR processing actually begins.

i am bit confused about the supporting documents .
In document check list its mentioned that

4. Proof of Principal applicant's relationship to sponsor:
- Photocopy of sponsor's birth certificate
if you are sponsoring the mother or father of your mother or father , you must also include the following :
- Photocopy of your mother or father's birth certificate

Question :


- i am submitting my birth certificate to prove Principal applicant's (my father) relationship to sponsor (me)

Do i require to submit my father's birth certificate as well ?

In your case, only your Birth Certificate will be fine. Parents birth certificate is required only when a sponsor is sponsoring grandparents.

WRONG, EI is not count ;)
