They didn't mention the MONTH in the TO and FROM dates i.e. it shows that I lived in Karachi from 1999 to 2007 whereas I requested them to mention November 1999 (I turned 18 in Nov 1999) to September 2007.
Should that be fine or I should get it corrected, although it will be very difficult for me because I live in Lahore.

It does not happen online. You need to ask a brother or relative in Karachi to do the process on your behalf. They will need to fill out the form, and submit it at the nearest station along with your passport pics (four) and your CNIC copy. They might need an authority letter from you as well authorizing your relative/brother to carry out this process on your behalf. There have been rejections in the recent past of people who only gave PCC from their current city of residence.
Same case, I cannot say the same thing for India. Pakistani PCCs include all temporary/permanent addresses that you have lived at by giving them the duration. So if you are stating in your application that you lived in city A and city B while you provide PCC from city A only then it will not be sufficient. Probably in India, the PCC clearly states that it is for the whole country. Even CIC says one PCC for one country.
I made one PCC from passport office in India and it went through without any issue.
