how to get consent form????

I am not sure if there is any form for that. But just a letter from the applicant stating that he/she is giving consent to the sponsor to get information about his/her application with applicants signature, name, file no, dob and passport no should do. When we applied for GCMS notes that's what we did. And also first time when my husband did case specific inquiry without consent letter they said they cannot provide any info without applicants consent.

I called CIC, their not giving me a proper answer their telling me they dont see any note of the first time requesting docs only the mail they sent yesterday, this is srsly taking away all my patience, i have a feeling this is all a blind game with cic of no clue for the situation of my file. I dont know what to do their like to be on the safe side re-submit the forms, so now this 48 day waiting starts third time for me

i understand , i wasted 3 months for submitting different forms.

Go to a different MP. MP of different area in the same city. My MP's office told me same thing on the phone and then I called another office in a different region within the same city and they told me to make an appointment and sign a paper so that they can do an enquiry on my behalf. Approximately 30 minutes of work I think. But have not completed mine yet, just been too busy.

You can order GCMS notes online. Any one living in Canada can order these notes. So you or your spouse, whoever is in Canada, as long as they have a Canadian mailing address, they can make the request.
You should order these notes if there is unusually long delay or you want to see if you are in the interview queue and the reason for it etc.

Easiest way is to go to this link:

1. Download the consent form IMM 5744. It should be signed by you and your spouse in India.

2. Now initiate the online request at top of link posted above. Fill in the information and proceed through the application.

3. When it says to upload the consent form, upload the above signed consent form.

4. Before submission it will ask about payment. You can use credit card. Fee is 5$

The process is now complete and you will receive notes in 20 to 40 days.
