Can some one provide me details on how to obtain the GCMS ? I did go to one of the past post, however the most confusing are the following :
1) section B and Section C., (i cant put my head around which section is for me(sponsor, living in canada) and which section is for my wife (being sponsored , living in India)
2) who signs where?
3) where to submit?
4) which is the fee line item, i could not locate that 5$ fee for this ?(any link?)

all suggestions are welcomed....

i got my GCMS notes today and it shows everything is passed, eligibility, criminality, security, interview waived, relationship genuine and passport forwarded to visa officer to issue Visa on 4 Feb which was In process date. but it shows awaiting medical results ? medicals were attached with the file when i submitted in missisauga? did they lost my wife's medical ..

I am struggling with it too, however, following are the details i know so far,

these are the two forms need to be filled in and signed

Hey samen, you need to be in Canada to apply or your sponsor can order GCMS notes for you. there are 2 forms you need to download, you need to sign one consent form authorizing your spouse to get information about your case from CIC.

Step by step:


Print off IMM 5563 (05-2012) E - Access to Information and Personal Information Request
Section A:
1. Access to Information Act
2. English/French (your choice)
3. Email (fastest way)
4. No (because you do not live in Canada you cannot request on your own, your spouse will have to officially request for you)

Section B:
This section is for you to fill out.
Family Name (Last Name)
Given Name(s) (First Name/Middle Name)
DOB
Electronic Files - Status of an Appliacation - File Number (you would get this from the AOR or Approval, ours started with an 'F') - Location (in your case its New Delhi, India)
DO NOT fille out the Immigratrion File in Canada section or the next section down that says Physical File

Section C:
This section your spouse that lives in Canada will fill out:
Tick the box "this request is usbmitted by a Canadian Citizian or PR"
Mr. Mrs. Ms
Family Name (Last Name)
Given Name(s) (First Name/Middle Name)
Full Address
Phone Number
Email
Requesters Signature: your spouse to sign and date

NOW:

Print off IMM 5475(12-2004) E - Authoriy to Release Personal Information To A Designated Individual
Choose One:
Tick off the first box "i authorize"
Questions 1-4 are for you, the principal applicant to fill out
1. Family Name and Given Name
2. DOB
3. Name of Office:New Delhi, India Location of Office: New Delhi, India
4. File Number or Client ID

Questions 5-6 are for your spouse, the Canadian citizian or PR to fill out
5. Family Name and Given Name
6. Address, Phone and Email (make sure to use the same email that you provided when you first submitted the application)

7. Principal Applicant to sign and date/Sponsor to sign and date

Pay the $5 online, print the receipt and fill it out with your sponsors information and email all 4 pages.

fill the forms out and then scan them and emailed them, the email address is at the bottom of form IMM-5563

it took 14 days to be exact .. i submitted the request on april 2nd and i got it this morning

You should see something like this in your status. Did you ever see the Medical results line before? Maybe you should followup with the doctor who did your medicals to make sure they have sent everything from their end. You can also write an email and attach a copy of the medical to NDVO.

We started processing your application on ***.

Medical results have been received.

thanks for such a detailed note but can you please confirm the following, I think there is an error in the description in section B and section C, the section B need to be filled in by the person who is living in canada and Section C is for the information of the person whose file is in question and for which GCMS is requested, I have been going back and forth with the form and still i think this is the way,, unless someone can correct me ...
and second i am not able to find the link to pay 5$ fees....

ok so here you go. if you are the sponsor living in canada
Section A
1. ---- Check - Access to Information Act
2. -----Check English
3. -----Check EMAIL
4. ------Check No for -Are you requesting information on your behalf and check Consent Included

Section B - filled by person in CANADA in this case your information
1. Check if you are PR /Citizen and check proof included ( Attach scan copy of passport or PR card
2. check your category [ i selected Business and 3. provided my company's name]
4 your Last name (sponsor)
5. your First name (sponsor)
6. your mailing address
7 Your phone number
8. your Email Address

Section C - this is for your wife in india (Principle applicant )

1. Her/his family name ( exactly the same as submitted in application)
2. Her/His Given name
3. Her/His Date of birth
4A . check Electronic file, Immigration File: permanent residence and next box file number with location (F0000XXXX and New Delhi)

Section D -
1. your signature (sponsor in canada)
2. Date

Attach -
copy of your passport or driving license or PR card showing that your are resident of canada
pay the fees online - its $5 .. and print the receipt
Ask your wife to print sign , date and scan you the consent form
Print the form

Scan all of them to PDF and email them to ATIP-AIPRP@cic.gc.ca
Subject : Request for ATIP for File # F000XXXX
Body :
please find attached documents for GCMS inquiry for
Principal applicant name:
Principal applicant DOB:

well i never saw the medical results received ...ever ..
but going thru the notes it shows under medicals

Medical : 1 ( which means they received the medicals)

UMI# XXXXXX( which is medical number from her medical form)
so they did receive the medical

For the 5$ link I am not sure as my spouse in canada paid the fee ..I will ask and reply for that ...But as far as I know U will find it on cic website where u paid for RTPF ..
1 more imp thing I want to add here ....
WE applied on 9th april for GCMS and got the following reply from them

I regret to inform you that we are unable to process your request for the following reason(s): We no longer accept the consent form you submitted. Please fill out the following consent form and send it back to us.

Your file will remain open for 30 days from the date of this letter. If you do not respond within this timeframe, we will consider your request abandoned and will close your file.

I had to resend the form they had requested ...so the consent form is now the new 1 ...

For the 5$ link I am not sure as my spouse in canada paid the fee ..I will ask and reply for that ...But as far as

during the refresh, Sidhu's reply did not apear for some reason, I see it now, Thanks for Sidhu and you as well, and about the fees, i went to the same link where we pay all the fees but could not find the 5$ fees, may be i am missing something, i will try again..
