I gave the guide another read... There is so much info that I forgot about that part. It's in the sponsorship guide. All they mention though is that it doesn't have to be "signed", nothing about it being filled at all or no!
 
Okay, here is what IMM 3900 says.

Sponsors living in Quebec

The province of Quebec is responsible for determining whether sponsors living in the province have the financial ability to sponsor family members and the length of the undertaking.

Sponsors living in Quebec or currently outside Canada but who intend to reside in Quebec upon their return must only complete the Application to Sponsor, Sponsorship Agreement and Undertaking (IMM 1344), the Sponsor Questionnaire (IMM 5540) and provide the required documents. Sponsored persons who intend to reside with their sponsor in Quebec are not required to sign the IMM 1344.

It also says this, in reference to IMM 5481:

Quebec residents do not need to complete this form.

Between the two, I'd say this is pretty clear.

Right, we are now in the last stages before sending the application. (At last!!)
I have got one more question regarding it: how did you guys send the pictures? Just loose in the envelope? Or in an envelope attach to the application?

Now regarding health insurance. Which company have you guys used? I won't get my "Assurance Maladie" (Quebec) for 3 months after my arrival and my husband won't have any until he gets his PR, which won't be before next summer. What would you recommend? And how expensive is it on average?
 
We didn't carry insurance on me, so I can't help there. (He's military, and long story short, we believed and are still relatively sure I had insurance through his federal coverage.)

For photos, all of the photos I wished to send (I only sent 8 photos, total, which stressed me out for a bit after I learned how much some others had sent, but we had no issues at all with our application) were digital. I put them all into a word document, with a paragraph or so of explanation typed under each ("This is from our first vacation (itinerary attached) to Mexico. We had never been parasailing before, and here we are, preparing to go for the first time." Etc.), and printed the four-page document on a colour printer. We didn't send any loose photos. I know methods of sending documentation vary greatly, and hope others will chime in with more of their experiences.

This is such a great idea! I wish I would have thought of it... Instead, we got 20 pictures printed and wrote a description on the back of each. ::)

You can use this website as a start. It compares prices/plans of multiple insurance companies. My wife paid around $50 a month or so.

I made what was probably a mistake putting them in loose. I should have put a rubber band around them.

For health insurance, we went to Blue Cross. It's about $100 a month, I think, but it depends on age, gender, smoking status, etc. They have an office on Sherbrooke just east of University. You can go to their website for a quote.
 
Excellent! Once again, thank you very much! I will look at both those websites for the insurance.

We are on the last stages of the application now... We had it all almost done but only filled half of the IMM5490 form (the longest one) before we had to save it as we had to go out. hen we went back to you, we couldn't modify or add anything!! So we need to start over... ::)
 
Hi guys,

On the form IMM 5669 question 4 "Immigration Office Requested for processing this application", what am I supposed to write? I thought there would be a drop down list, but there isn't... We are currently living in Manchester (England) and moving to Quebec on the 03rd December.

As you are applying outland through the UK, the office is London.

We are posting the application tomorrow! At last. One month later than we wanted, but at least it's going!

I made the payment online this morning using my Canadian credit card, for which I have a correspondence address in Quebec (my brother's address). I printed the receipt as requested, but where they ask about the "Payer Information" (name, address, phone number...), do I write my current address here in the UK or the address of correspondence of my credit card? The reason I am asking is that the telephone number section is set up for a Canadian phone number. Thanks again! This should be my last question for a while... ;D

I had the same situation, Canadian credit card had a Canadian address. On the receipt, I wrote my current UK address, added +44 in the area code box and made the UK number fit in the spaces :)
