OK, so the form that your hubby took to the doctor would have been the Appendix C from the Regional Guide. On completing the examination, the DMP would have given him a smaller green/white form with his photo on it, as mentioned by Jamgirl, which the doctor would have signed and stamped with a maple-leaf stamp with his DMP number on it. THAT green/white form is the IMM1017. This should have been attached to the application, as it is required by the document checklist.
I see from the last update list that you already received sponsor approval and your file has been transferred to KG, which means that you likely did attach the medical form to the application. Otherwise, your application would have been returned to you, because proof of valid medicals is an essential component of a family class application. CPC-M usually sends back applications if something is wrong with the medical (AliCat had hers returned because the first medical they did was expired by the time the application reached CPC-M).
However, IF you still have the IMM1017 for some reason, get it ready with the package of additional documents and send it to KG, ASAP. Don't wait for them to ask hubby for it. Make sure you include a cover letter showing you have it in the package.
