Oh Lord...help us to get it soon
Yes, you need spouse authorization for release of information when applying GCMS for the first time. You can use a form for data release or appointing yourself as representative for your spouse - both are available at CIC website.CECAUG1 said:Hi guys,
I need some guidance. Being a late July applicant I am getting impatient now. I am thinking about applying for GCMS notes. I applied from Me, my spouse and my daughter together. To apply for GCMS notes:
- Do I need my spouse's authority or some kind of proof too?
- Also I applied through a lawyer, so can I apply for GCMS notes or only he can do that on my behalf?
Please suggest?

You can apply by yourself, you do not need your lawyer for that. The request should be under privacy act and its free of charge.
Yup, that's the one.CECAUG1 said:Hi sashali78,
Is this the right form? Consent for an Access to Information and Personal Information Request – IMM 5744.
I found at http://www.cic.gc.ca/english/department/atip/form-imm5744.asp
Please suggest.

That's correct...you do not need any consent if you're primary. By default all your dependents info comes on the GCMS notes.Hopefulagain said:Hi CECAUG1
NO, you do not need your wife's consent if you were the primary applicant. Your file will contain you family's info too.
