Check the portal. Did you apply under "Gaining Experience"? There were two categories "Gaining experience" and "Direct to PR".
Given that she already worked 24 months, she had to apply for "Direct to PR".
Offer letter is NOT on the document checklist of Direct to PR but is on "Gaining Experience". Only proof of experience is required (unless the experience was not genuine).
The direct to PR is still open. You can re-apply with proper documentation. You will need T4s, Pay stubs, reference letter, etc.
Speak to a lawyer, it could be mistake of officer.
