There is no process. IELTS as per their policy only provides one copy of TRF. I had a long fight with IH London (exam center) thru numerous emails and phone calls still their staff members were clear that they don't issue additional copy. However, it's mentioned on their website that people applying to CIC or UK BA would get an extra copy here:

After I send them this link and bombarded with number of calls and emails they finally agreed to provide an extra copy. Again, answer is there is no defined process. Your only option is to talk to exam center and fight it out.

Google is best option for cover letter formats, and it's final structure depends on your profile, you can address it to CIC. There are no fixed set of important points, it's entire on your circumstances, clarify points you consider to be potentially confusing or complex. If any info/document missing then explain the same. Overall your letter should go thru all points which confirm with reference to documents your eligibility for your application!

Well you can use your 10th or 12th class certificate if it contains your parents( both Mom and dad ) and your name including your Date of Birth , with an affidavit which contains the explanation why are not able to submit your Birth Certificate .
