Does anyone know the process of getting additional IELTS TRF from IDP in India (Delhi)?

There is no process. IELTS as per their policy only provides one copy of TRF. I had a long fight with IH London (exam center) thru numerous emails and phone calls still their staff members were clear that they don't issue additional copy. However, it's mentioned on their website that people applying to CIC or UK BA would get an extra copy here:

How many Test Report Forms (TRFs) am I entitled to?

You will only receive one (1) copy of your Test Report Form, unless you are applying to Citizenship and Immigration Canada (CIC), or United Kingdom Border Agency (UKBA) – in these cases, you can receive two. Proof of application to CIC and UKBA must be provided.

After I send them this link and bombarded with number of calls and emails they finally agreed to provide an extra copy. Again, answer is there is no defined process. Your only option is to talk to exam center and fight it out.

how critical is the issue if u had forgotten to write your full name at the last page of generic form..... ( i missed it :-( )

It is not critical; but if you can fill up 100% accurate forms, you will feel mentally better.

Please help me here. My DOB certificate mentions my surname, however i have never used it anywhere (not during education, not in passport).
Now in CIC application i need to provide my DOB certificate. How to handle this situation where my full name on DOB is not going to match with my rest of documents.
Do i need to provide Affidavit? If possible can someone provide me format for Affidavit.
My location - Punjab, India.

I am 7th May applicant and my CC was charged on June 13 - One Month ago and till now I did NOT receive the PER !!!!

No one knows how CIC works and on what priorities

Yes...affidavit will solve it.
Format....it differs but just tell ask in your location that you want to do Age declaration affidavit/Sworn declaration of age,and they will give you their default format.

You are not alone in this scenario.
CIC will soon commence the distribution of PERS notifications to applicants affected.

Well you can use your 10th or 12th class certificate if it contains your parents( both Mom and dad ) and your name including your Date of Birth , with an affidavit which contains the explanation why are not able to submit your Birth Certificate .

Is cover letter mentioned on document checklist? - No. That means it's not mandatory.

Google is best option for cover letter formats, and it's final structure depends on your profile, you can address it to CIC. There are no fixed set of important points, it's entire on your circumstances, clarify points you consider to be potentially confusing or complex. If any info/document missing then explain the same. Overall your letter should go thru all points which confirm with reference to documents your eligibility for your application!

Dear Future Applicants:

Before submitting your application, do the following last moment check: Most Frequent Errors​

• Ensure counting all the pages of form.
• Ensure writing your full name at the last page of generic form.
• Ensure selecting correct visa office from http://www.cic.gc.ca/english/information/offices/apply-where.asp
• Ensure writing names in native languages
• Ensure writing name, DoB, photo taken date at the back of photos
• Ensure including self-addressed labels
• Ensure while filling form IMM5406 by your spouse, s/he is applicant
• Ensure Assets = Your property + settlement fund
• Ensure Settlement fund is owned 100% by you or your spouse or jointly by both
• Ensure providing original ECA, IELTS, and PCC
• Ensure providing photocopy of degree/master certificate
• Ensure birth certificate contains applicant's name, parent's name, and DoB
• Ensure using DD;
• If using CC, ensure diligently fill all the field of Section B of payment form: amount, card holders name, signatures, card numbers, CVV, expiry date, and type of card.
• If using CC, keep it inside a vault.
• One application, only one intended NOC;

• Ensure getting 67 marks.
