Make a parents declared affidavit and send along with the copies of old BC and 10th passing certificate.

Does anyone know if married and husband's name is not on passport. Would CIC make that an issue when it comes to passport request? Is it mandatory to update passport name if married?

This isn't an issue, though preferably the spouse name should be updated..

one question,

for some of the forms, like in representative form : Address : PO BOX / FAX Number and then Spouse Signature / Representative ID number etc.

these sections were not relevant for me so i left them blank and attached a sheet explaining why i left these blank.

can you guys confirm if this is the right thing to do ?
