1. Affidavit by both parents preferably. Affidavit by self is not valid. Incase parents are not available, affidavit from Persons who where around during the time of birth, and preferably a close relative in that case.
2. Explaining you tried all means to get a fresh copy of BC, and why couldn't succeed.
3. School records showing date of birth, including junior, middle and high school with both / either of the parents name.

CIO will ask you for any changes later.. before they issue the Visa.. So before that get an affidavit for name change and complete the formalities at your end...

Cheers. I would just skip the national ID altogether.

Now my application is ready. However, posts on this forum suggest that the cap for NOC 1112 has been reached. Would you suggest applying now or wait till Jan 2015? Im worried that rules would change by then....

Self Affidavit should be fine. It is a legal document and has sanctity. We can substantiate that with any other proof with parents name e.g: Passport.
