The form asks whether you have used any other names. That is certainly another name. However to avoid this appearing to be an alias (which can have negative connotations) I suggest explaining in an annexure the exact situation and that it is not an alias.frasiercrane said:If the name appears on any document that might be required by CIC; I advise to put alias.
