The SSC includes DOB, but there is only 1 name:

Royston Dsouza Francis.

Where Royston Dsouza is my Name and Francis is my Fathers name. But there is no special row which mentions Fathers Name: Francis Dsouza.

Will that do?

I guess you have provided affidavit by your parents. That will save you.

1. In many forms, space provided is nor enough. Guide 7000 says "Download and fill out the application form on a computer". Can we fill some parts of the form with pen?

2. Can we use office address in form 5406 section A and form 5669 para 12?

Hey guys
My application was received by CIC on 4th June.
I didn't include the affidavit of the translator who translated my Birth Certificate..
I didn't knew earlier that we need to submit it, However the translation is notarized.

Is it worth sending now? I am pretty sure that they won't accept any additional document as i don't have the UCI.
But is there a slight chance or has anyone sent any additional documents that has been accepted without UCI.

While filling the WES evaluation form, there are multiple options in INSTITUTION TYPE eg. State board of Technical Education and Higher Education.

Can some one clarify when to use the State Board of Technical Education option

Kindly recheck, if you are applying for Canadian or US ECA. WES operates in both countries, but u need canadian ECA for FSW (even regular Canadian ECA may cause issues), and technically it should give you Canadian address.
