I think so yes however as per instruction guide, it does not say about attaching copy of PR card.

To be safe, send photocopy of passport biographical pages and photocopy of both sides of health card(it should have your name and date of birth) and attach PR card copy as well (both sides).

This is what it says on the website:

5. Two (2) pieces of personal identification

Both pieces of identification should show your name and date of birth, one of which must have your photo on it.

Examples include:

a Canadian driver's licence
a Canadian health insurance card
a copy of the biographical page of your passport/travel document, etc.
If there is information on both sides of your personal identification documents, provide a photocopy of both sides.

Note: Social Insurance Number (SIN) cards, bank cards and credit cards are not acceptable pieces of identification.

OK. That surmises that Passport and PR card should be enough. I will also send my health card from previous province. But Passport has a 5 year old Canadian address and the PR and Health card dont have any address on it.
The only card that has picture and address is the Driver License, but I will lose it immediately when I surrender it at the new province.

I am already submitting PR card and Passport biographies. Is that enough?
I am applying from a new province/new address, so I will be able to send only the old provincial Driver License ID. And the health card doesnt have pic on it. Waiting for new ones will be between 2 weeks for DL and 3+ months for Health.

Please send your driver license and health card. In addition, attach a letter explaining that you just moved to a new province (for work reason) and you applied for new driver license and health cards with new address. A receipt or letter from your city DMV and/or from your province healthcare center will be a good reference.
Two reasons why a copy of your passport is not the required ID:
1. A copy of your biographical page of your passport has been asked somewhere else
2. I am sure that your actually passport is not delivered by Canadian gov.
I did not submitted my PR card as a prove for my ID because they already have it in their system and it will be twit to ask for it again. In summary, give them the IDs that you got from our government and CIC does not have yet.

Agree. With CIC more is better than less... you always want to be on the safe side and don't give them any reason to return your file. Your Driver License and Health Card are still valid... You will probably be on the safe side by following the above suggestion.

My 9 years old has no photo ID except his passport biographical page (OHIP has no photo and no school card issued). Do you think it's enough? CIC site mentions a school record...any idea what that is?

Bio-pages of passport with photo + PR Card with + Health card without photo + school record with or without photo Should be enough. Just add a cover letter explaining the situation. I did that and application was accepted and is in process.
