The medicals should be done BEFORE sending the application to Mississauga, as it is part of the application.
You need to select a doctor in the list from CIC, get an exam from them, and they will give you a paper to include in your application to prove that you have done it ( not the actual results, but only a proof).
If you haven't done it - start getting the information, phone the nearest doctor, and book your appointment. When CIC approves your file for sponsorship ( if they don't send it back for medicals missing, sometimes they do, most of the times they don't), they will give you a file number. Then send the medical receipt to your VO in Amman and you can send them the proof of your medical. You can wait until they request it ( as your VO has quite a long waiting time) but it should have been done with the rest of the application in the first place, so it will delay your file (because then VO needs to issue the letter, send it to you, you do the exams, and the VO has to wait for the results to arrive from the doctor).
