For the proof of medical examination for the sponsored person, the checklist asked us send Copy 1 of Medical Report: Client Biodata and Summary (IMM 1017) However, the doctor in the clinic says they don't issue that. In the end we only got a small piece of receipt from the clinic.
Can we send the receipt for the proof of the medical examination? If yes, do we have to send the original one or just the photocopy of the receipt?

Was it a single sheet of paper with a picture of your spouse and a "UMI" number on it? If so, that's what you need to send. Ours had a section at the bottom saying "You have completed your health examinations prior to applying for a visa. Please make sure that you attach this letter to your visa application before submitting the form to Citizenship and Immigration Canada."
It confused me as well because I kept looking for something that said IMM 1017 on it.
