I am ordering my notes, I just wanted to know in the upload document section, I am scanning and attaching the consent form signed by my husband .. do I ned to attach one signed by me as well? Also, which pages of passport should I scan for the proof of citizenship part?

I am not missing anything now after this am I? I fill out the online application with ATIP request and then upload my documents, pay by CC and I am done? Right?

Jerry, You will sign the same form that your husband signed. You do not need to attach your passport copy. Why are you doing that?
