i went through the form too... and i guess now we will all get the requests to fill this form out too... and maybe the ppr is now delayed till feb.. or they can send us ppr and this form request together ...

man I hope not, I don't want to wait another year for this stupid garbage. So what the hell should we do? what if I sent this stupid fucking form now? would that be a problem? I think I'm going to have my wife send it just to shut them up and keep processing my file.

call the stupid doctor you dealt with at IOM and ask them if they've sent it and ask for tracking number.

yes there is bcz if they stamp it on friday too, so u will get it on saturday through TCS

people say not to send anything urself if its not requested.. so lets just wait and pray... who knows we might not get this form, may be not in wife cases... or with ppr maybe..
whatever it is, i just want this torture to end ...

I asked my husband too to ask the doctor about getting the tracking # from Dehli Doctor but the stupet doctor didn't give any tracking # , except the receipt and told my husband it will take 4-5 weeks or so to process, however it has been over 5 weeks now

Yes you might be right . I got an email from CHc about my husband case which is in their office since November 2009 but still they reply like this:

Dear Client,
This is in response to your email (see below).
Your application is in process. No further action is required by you at this time.
We thank you for your patience and for keeping your correspondence to a minimum.
For future reference, in all correspondence to our office, please refer to the applicant's full name, date of birth and file number.

yea Allah help us all , I am too frustrated as well can't wait any longer. I am just tensed if my husband doesn't get his visa till November 2012, he is telling me to come over for visit but if the ask me about that residency form I don't know what to do with it.

I think everyone is pre-panicking a little bit here.. I might be wrong.. but just because yasirman was asked to fill out the residency questionnaire it doesn't mean all of us will. I think the reason he was asked was because his wife resided in Pakistan with him for multiple months and then went back. So CHC having this information is questioning whether they will settle in Canada or not. I haven't seen anyone else who has been asked to fill out this form. Don't worry InshAllah next step for all re-medicals will be PPR not this stupid form. I hope and pray that, maybe I'm wrong too but just pointing out that it is very likely that next step is PPR so keep your spirits up folks!!

the same thing happened with me, i submitted my pp on 24 se, and on 25 sep there was written that we start processing ur application on 15 sep, 2012 .
but on 7 oct, it becomes decision made and address showing canadian address ..
so dont worry inshallah soon ur status will be decision made.

only yasir... its added in document checklist, who ever would apply now has to fill this form in the beginning only... so now its kind of mandatory, so i guess the people who has not get the ppr yet has to fill this form before they request their passports,,,

Previously too people have been requested this questionnaire with other documents. they only ask for additional proof of relationship from some people with whose proof they are not satisfied. Similarly they ask for this Resident Questionnaire from those who they suspect that they haven't been living in Canada permanently. They have made that compulsary because many people leave canada to live with their spouses in Pakistan for a longer period. That complicates the things for the applicant as they are required to have the intention of living in Canada for the rest of their lives.
