all the forms on the right hand side has to be completed and signed by you and your spouse, then you can scan those and email to the address available on the same page ........... it takes about 5 weeks for them to release the notes there is heck of a backlog in their office as the assigned lady told me ...... keep checking your credit card transactions, once you see they have taken $5 from your credit card then in about a week you will get the notes by email, make sure you keep a strong password for your email because the notes have all personal information .... once you receive the mail download it, print the notes and then delete the mail from mailbox and also your computer ........ I am a bit overly obsessed about internet security
