But after waiting 03 months I had not recieved any AOR from London Visa Office. Then I sent them an enquiry asking about London Office File Number so that I may correspondence with them in future. In response they gave told me that "your file has been placed for next stage please have patience and your previous file number is your London Office File Number" and after having same file number i again checked my e-case status it has been changed as "Decision has been made" :-( where as 1 month back it showed "Visa Office has recieved your file"
Please can you guys tell me that an early decision means "REFUSAL" or may be some good news. As it's been just three months and they have taken decision.

Its not a good news, decision made before medicals is a rejection it could be a system error as well but if its not then mostly likely the reason is that you dont meet the minimum points requirement so you failed on the first step, you willl receive a letter in 2 weeks time.
I hope its a system error!
