Thank you all but am very under confifent guy so i called my consultant’s assistant if he could looking into my file before i submit and he agreed but then told me questionnaire is ok and form has lots of mistakes... and i should better go through office... idk if he was being genuine or was marketing himself . Hello, I took a look into your images and I would do the following considerations/corrections:
- For "Personal Details" - item 1: Make sure your "Family Name" and "Given Name" matches what's written in your passport. The "Family Name" item would be the same as "Surname" (at least for my country's passport, I googled for passports from India and it seems to be the same pattern). Observation: These fields (Family Name, Given Name) should also be the same as it's registered for your CIC account (you probably can check your current permit to see if it matches).
- For "Current country or territory of residence": the "from" and "to" should match the date of "issue" and "expiration" of your work permit. If you had 2 work permits in sequence, it should be the date of issue of the first work permit and the date of expiration of the second work permit (and you should do a note about it in your letter of explanation, and also attach the old work permit photocopy besides the current work permit). Observation: A coop work permit wouldn't be included in this logic as you would be a "student" and not a "worker" with that permit. An example of 2 work permits in sequence, is 2 post-graduation work permit because the person had an old passport that wouldn't allow him/her to take the 3 years post graduation work permit [so the first wp goes until the expiry date of first passport, and the second wp attached to the new passport goes until the person completes the total of 3 years of post graduation work].
- For "National Identity Document" it shouldn't be the Canadian Driver's License. It should a national document from India (even if it doesn't have an expiry date, in this case you just let the "Expiry date" in blank and you make a note about that in your letter of explanation). If you don't have a national document in India, you can say that you don't have a national identity document and let this section in blank.
Observation1: if you have a national identity document from India you would add the information here, and attach a photocopy of the document and a certified translation. Googling about it, it seems there's a document that can be considered national Id

Observation2: if you use the Driver's License, and have a correct passport information in the form, there shouldn't be any issue even if the officer consider the Driver's License not a correct national Id (this is my impression, I don't have an official information about that, but I already used Driver's License for national Id for a TRV application and it was approved, and I also used this information in my PR application and the documents were accepted).
- For "Details of Intended Work In Canada". Make sure to select "Open Work Permit", and it would be correct the rest seems correct. However, what kind of PR application have u done? If it's through a Provincial Nomination, you would need to select the "Province" and the "City" as well and check "yes" for item number 8 (Have u been issued a certificate...).
- For "Education". On "Field and level of study" I would fill "Business, College Diploma" but I don't think it's wrong the way you have filled (maybe it seems a little strange to say 2 years college diploma, and there's no 2 years in the "from" / "to" fields).
- For "Employment". Move item 2 to first position, and item 1 to second position. Your most recent job should be the first entry.
- For "Background Information" item 2-d. It seems correct to me the way you did (maybe just a semicolon or comma to separate the first application from the second). I would fill like "Study Permit issued yyyy-mm-dd (application number), Work Permit yyyy-mm-dd (application number)" because I have more other application to quote.
- For the "Signature". I'm not sure you typed your name in the Signature spot. If yes, you should let it in blank because before you submit the documents there will be an option to do a digital signature through their website in the cic portal.
Anyway, if you can have some professional look into your forms I would advise to do it just to make sure you have your forms and your "Letter of Explanation + documents attached" pretty good to go and you can have more "peace of mind". I would recommend this immigration consulting company
