What you should be doing right now is to raise a Case Specific Enquiry through the Webform. You can do this by yourself and this is the best way to reach IRCC!! Write a clear message indicating that the Police certificate was in fact uploaded and it was probably a human error by the Visa officer.
Even if you reach them by phone, they are going to ask you to raise a CSE!! Make sure you include your UCI number, this should be on the top of the rejection letter you received and express entry application number and other details. You will also be able to upload a file in the end of this process and if you so wish you can at a strong letter with your signature.
Do this ASAP!!!

So his lawyer raised CSE. After 4 weeks his application was reopened and he was asked to re-upload all of his documents.

The PCC for both applicants were uploaded. The dates mentioned in the rejection specifically apply to me (based on the year mentioned - 2009). Both PCC (FBI and Indian Consulate) do not provide specific dates and addresses. Regarding the cancellation, did you upload PCC(s) for your dependent, as well? And if so, did the dates mentioned cover your dependent's address history correctly?
