Hi guys,
Today i have sent an email to cic bcz they sent me medical form on my father's name instead of my husband' name. While i have informed them about my wedding and also sent copies of updated passport and nic which is on my husband's name.
Due to difference of my last name, i am unable to do medical so plz pray for me.
And i hope cic reply me very soon with correction.

I think if the name is same on passport u can do it, because they only check original passports..

@sani being old member i have seen pple making little correction at the time of landing also i think u shd go for medical and this mistake may be corrected after medical by sending email or even at the time of PPR

But they did not give me appointment bcz my passport has different name from the name of form.

yes they need to be same... you should write to LVO as an urgent matter... you will hear within a week from them...

I have sent an email to Lvo and i hope that i'll get new correct form very soon. Just all of u pray for me.

It seems that their computer system uses IM0008 form data to generate IM1017 form automatically (Remember bar codes at the end of IMM008 forms that contain all information you fill in that form). I do not know if they updated the information you send later into their system. IOM should not refuse the appointment as medical tests information uploaded into CIC system is by IME#. Passport is just for your identification purpose as far as IOM is concerned and if your old passport is also attached with new passport, it should not be problem..

I have sent updated IM 0008 on 17 march 2014.i have my old passport but that was cancelled by passport office at the time of renewal. I have imm 1017 and that have my uci# and application number as well. So do u think that my medical can be done?

As far as CIC is concerned, it should not be problem as they will definitely update your record. Did you email CHC London office regarding the IOM appointment issue. They will definitely reply you very soon as they replied one of the guy within a couple of days whose OWP medical was still valid and they sent him another medical request for PR processing. I remember IOM did not ask my name on IM1017 form while making appointment. They just said that I need to bring IMM1017 with me on the day of medical. You can make an appointment with IOM with name on new passport and hoping that you will get back the reply from London office. This will save wait time for appointment
