My husband got his done at the police station and all he filled out was the box for citizenship and his social security number. He left the OCA, FBI no. and MNU sections blank

yeah, mine was sent within days of the expiry, i had no idea it was only valid for 90 days. I got another one as a backup just in case it was ever requested, and never needed it. i think if the report was already "expired" before it was sent in, then they would request another one. otherwise, yes, it would be up to the visa officer if they want to bother. my guess is they can get away with records that are close to the date of application, not ones with larger gaps of time
 
I agree absolutely as big things could happen between long periods of time. Better be safe than sorry.
 
We are trying to plan out her medical, she lives on Long island NY, and going to the city is a bit of inconvience are you only able to get medicals done with "panal physicians" and is she able to get them done in Ottawa when she comes here instead? if so will she get the results that day? if not can it be mailed or does she have to pick it up and how many days would that be?

any advice or input appreciated

The medicals must be done by a panel physician. She can get them done in Canada if she wants (it's cheaper from what I've heard). They should give her a receipt, which she then sends with the application. They don't actually give you the results.
 
We have sent in our fbi background check about 3 weeks ago, we sent the payment info I think it was 18$ and we havnt heard anything back like comfirmation letter and the money has not been taken out of our account. does anyone know if they send you anything letting you know they recieved your request and everything is in order? and also how long till they take the money out?

no, they won't send any confirmation.

i think they take the money out when they start processing the record, which is taking a long time right now. there are recent posts from members reporting information from CJIS about the processing time. i think they just started processing requests received in november.
 
I sent my FBI clearance request on Nov 10. I haven't heard anything from them yet. They did take the money from my credit card on Jan 24 though. I sent my application in on Jan 29 without the clearance. I typed up a formal letter explaining and telling them I'd send it in later and I also included a copy of the completed application form that I sent the FBI as 'evidence' that I had in fact submitted the request.
 
Yet another issue, sorry for all the questions and we really appreciate all the help from the forum!

We will be getting married in march, but we booked our honeymoon in june and we used her last name for the tickets so we wont be able to change her passport name till after. We are wondering what are we suppose to do about sending in our forms what names to use and if we have to use her last name is it going to be hard to change it to mine after ? or can we put my last name on all the forms and hopefuly they dont request her passport till after we get back and get it all changed which will be about 2 months after we send everything in.. if we cant do that is it going to a problem for her if she changes it later on but all her forms are in her last name but her passports in mine?? sorry I know this post is all over the place.. any insight would be appreciated!!! also if we she puts my last name on the marriage certificate is that going to affect anything??
 
The information in the forms must be true as on the date you sign them. If she changes her name while the application is still in process, you/she will need to inform the change to processing office. It won't be a problem.

I don't think she can use your last name instead of hers on marriage certificate, not sure. May be someone else can give you a correct answer about this.

Thanks for the reply, how would one go about informing them on making changes? is there a number or will we have a case number and place to mail to and is there going to be new forms for changes?
 
A letter informing the Change of Name along with official/legal evidence. You will get a File Number for your application, generally. after 1st stage approval.
 
We got married in December and I was required to use my old last name when I signed the paper after the ceremony. After that, I started using my new married name.

Did you send a Personal Check? FBI does not accept personal checks.
