I asked about that, and told them I got biometrics done at USCIS office, they said I could leave the last 2 pages blank and only fill out and sign the second page.

Just got off the phone with them, and they said 'Original Passport Submission'. If in doubt, feel free to contact them and ask. The wait time on the phone was only around 5 minutes before being connected with an agent.

Yes you can. I did that because I haven't heard of anyone who submitted it there, and just to be safe. But under the Application Type, I chose 'Original Passport Submission'.
