Yup, mine is blank too (box 36). Not to worry though. I had ordered my CBSA report and it stated my original entry date of 2006. Plus they have my study permits etc on file, so I'm sure all will be well.

I'm sorry to say that I was part of that new program, where they moved my file to the Ottawa Pilot Program from Buffalo. I remember folks who applied before me, got their PR, 9-10 months after I did. :-[

When I print from Chrome, it gives me the date on the top left for July 26. Will that matter, or should I do it on August 1 itself?

You're completely wrong about this. Just because they don't stamp your passport, doesn't mean they don't keep a record in their systems. Your PR card and/or visa is swiped everytime you enter the country. Even when signing the Citizenship application, on the last page you give CIC the permission...
I think if you don't have a surname, then leave it blank. If you right "NA or Not Applicable", the bar code generator will assume that "NA" or "Not Applicable" is your surname and will generate a barcode based on that. You don't want that to happen.
