You MUST have ECA done if your qualification is outside Canada.

Are you in Canada, working in that field? If you are not in Canada working in the field, you do not need the license (I assume that is what you meant). My profession is regulated and am outside Canada, it is not needed until you get there. You don't need Medicals for OINP. You need medicals...
