Yes. You need to apply for that. You need to get a name change certificate from the provincial government, change all your names in your health card and driving licence and then send in the all the details to change your name in the citizenship certificate.

As far as I know, they will put the name thats in the citizenship document and its always good to have your name changed now rather than latter. Its just my 2 cents.

I had the same problem like you. The only good way is to apply for a provincial name change. Get the provincial name change document and then do the name change in your driver's license and health health. Get all these 3 documents notarized and send a letter to CIC stating your name change. I...
