RQ - Clarifications Please - From NS

Hello,

I got RQ for my Citizenship application and want to know the following.

1. I do not have rent receipts, so what can I do for that?

2. How to get the health records? I have lived in 2 provinces. (NB and currently NS). How to obtain this one? Do I really need to submit the health records, I am asking this because, I have not used any medical facility in Canada from the day I arrived here.

Currently, I live in NS and my application is processed at the Halifax CIC office.

Could you somebody help me in this regard please.

Hi NS_SJ,

If you have paid rent with cheques you can get copies of cheques from your bank,attach them with the lease agreement. If rent was paid by pre-authorization and deducted automatically from your account, you can have the bank statement and highlight the rent payments.

In addition to moti's comment:
1. if you have online banking, you can view and print the cheques off it(check to confirm it is free, some banks charge for it).
2. To obtain your health records in NS, contact MSI and find out how even if you've never used the service. Submit whatever you get from them

1. How do you pay our rent? cash, pre-authorized debit, cheque?
2. Do you have a lease?
3. Are you registered with a doctor? have you had any blood work done, physical/ medical exam?
4. Do you have health cards?
5. Have you been to the dentist? Optician?
6. Are you employed? Do you file taxes?
7. Do you volunteer e.g soup kitchen, library, school?
8. Have you travelled in the 4 year period? Do you have passport stamps with exits/ entries as appropriate.

You need a trace of physical presence in Canada...if not its going to go the Citizenship Judge with a chance of refusal.

I admit that I'm quite proud of locating the health record information. It was a challenge because every province and territory calls the form by a different name and has a different process for accessing it.

If all I did was go 2-3 times to the clinic and then once a year for flu shots, following the process for ON will give me a short summary of the few times I accessed the clinic - yes ? Some form of check-in summary or info ?

Also, as is quite common now, like many I have not been lucky with finding a Family Doctor so I can have the annual physical. So all I would get when I request OHIP records from ON is a basic listing of my use over the requested time frame ?

For Ontario, the Personal Claims History (PCH) has four columns:
1. Your health card number
2. Your name
3. Service Date
4. Provider (by six digit code)

The PCH does not describe the nature of your visits to health care providers. It lists all health care providers that you visited, including family doctors, walk-in clinics, emergency rooms, x-ray or diagnostic facilities, etc. If you received more than one service during a visit (i.e. flu shot AND a prescription update) there will be two services listed for that day. If you had to go to an emergency room, there will likely be ten or more services listed for that one day.

Because the RQ specifically requests it, it is worth getting even if you did not go to the doctor very frequently.
