Hi JJ-C, when i went for the medicals, the doctors office just asked for the passport and the form immigration sent me. I did not provide them with any of the file numbers. I guess they will submit it for my CEC file, the one I got the medical form for. I also read somewhere on this website...

I know , happened with me too. but luckily i checked it on laptop first and on i phone later to make sure I actually have received the medical form finally. ;D the one on i phone was blank.

hey paul, When i got rprf n medical request, the email stated that an email with medical form and instructions will be followinh the previous one. and I received my medical form after 15 days after emailing CIC twice. The form did have my name, address,email ,dob , file no and also a barcode...

May be wait for 2-3 days and then contact them...what do you say?

Just received my medical form :D from CIC

I am just browsing through the CIC panel physician program and the forms the physicians have to fill contain IME # and UMI # ...may be that's what the doctor's office creates...not sure though...this is the link http://www.cic.gc.ca/english/department/partner/pp/

She said she'll create an ID for me and if it doesn't match with the ID created by CIC... they'll charge $25.00 to merge it. They might be able to merge barcodes but creating a UCI should be illegal. So I don't know. I think I'll go to the other doctor. Terki n Jayson...what do you suggest?????
