Im just a bit confused if someone can please clear it up for me. Form 5406 additional family information. at the start it says: Complete ALL names in english and in your native language..... So my native language is vietnamese. Do I have to write all names in english, then followed by or below...

can someone please verify.my wife is main applicant,from vietnam.the photocopy of her passport does not need to be translated or notarized.is this true?also my canadian passport photocopy does not need to be notarized? Thanks.

She has to sign it personally in front of a notary.

Ahh ok. I did live in California for 2 years and came back to Canada sometime in 1998. Now problem is, I dont remember the exact date I came back to Canada,it has been so long. How do I answer this? Do I just write on a separate paper explaning I dont remember the exact date? cheers.

I think i have read in these forums ppl are saying that emails,text messages, you can have a friend translate it, or even do it yourself.I translated 40 pages of text messages myself and highlighted significant events. I think you dont need a certifed translator for such things.Just for those...

i got same situation as you. Just leave it blank, i did. I called cic centre, they told me its ok to leave it blank,they can find it, no problem.

Visa office Singapore. When the sponsor has been approved. Does their application get sent to Singapore along with main applicants application? In other words,does the VO in singapore also review and look at the sponsor's application?
