I think a photocopy of the new one would suffice, they will be able to link it with your application through your UCI number. That's what I did during PR process

Just an update about question number 5. Another time I called a different agent said to leave it blank if you haven't used any other names so I don't know anymore. I guess as many opinions as telephone agents.

Call them before you submit anything. They might say it's not even necessary.

If they say that it's better to update the info you can always submit the web form, explain the story and give them the missing info.
