Depends upon the letter requesting passport copies. Only thing you have to do in sending two passport copies is to scan more pages. I would send both.

You can send the information by webform after you get the birth certificate.

Where was your parents' medical done at first? Is it possible that your file is still processing in CPC-M but the review of medical results are done from Delhi. It is odd that you did not receive any communications about the file transfer.

IMM0008 is required only for the principal applicant (PA). Information for dependent has to be added in the same form of PA. Better get help of experienced person or the immigration consultant.

Did NDVO request only PCC? No other document requests? Normally they request other documents like travel history, military history etc.

Didn't you receive SA letter? LVO is mentioned in SA letter.

MyCIC page must have UCI of PA and dependents. Biometrics collection letter and medical form also has PA's UCI.
