I just followed instructions of letter, irrespective of what I think I did. My letter asked us to fill it out correctly and we did that. Id suggest follow the letter and fill the form to the T, clearly, sign and date the forms where it asks you as per their instructions given.

you can look into which email address you put in the application. It was found that they sent an email to my dad, and he didnt check email for a few days. Its risky to miss the email. Check all possible emails, junk mail folders, etc. If you had a mailing address only, check your mail box! Good...
