As Charlie said, application fees are $1040. Pay it all up front to avoid delays later on. The police certificate costs I think 35 GBP. Make sure you order this closer to the time you are planning to file. For a medical, I paid $180 total in Toronto. I did get a quote from a DMP in the UK...
No, send a photocopy of your passport and birth certificate. If the UK is not your home country then also send copies of any immigration documents etc.
You can also try using the receipt number and the sponsors details, but again, you won't be able to view any info until around the time they are working on your application - 75 days according to website (although it says they are still only working on applications from 90 days ago!)

You will need the COPR, passport and list of goods. Are you having any things shipped over at a later date?
The list of goods accompanying, don't worry too much about it. You can write your own general list or use the form, both will suffice...

As long as when you landed you did not qualify for common law status i.e. living together for 12 consecutive months and you were not married, there will be no problem with sponsoring your spouse now.
