Hi,

I'm applying for PR under CEC. My wife's school certificates has her name as ABC LMN XYZ, where LMN is her father's name. She is now using name as ABC XYZ (last name is still same, so only difference is removal of her father's name) in Passport & marriage certificate etc & we are applying for her PR under this name.

My question is - do we need to provide any supporting document (e.g. affidavit) for name change? If so, can anyone please provide format for that?

Are you planning to provide your wife's birth certificate? If yes, and the name is just the first and last name, then you are fine. School certificates are just additional documents that you can provide in lieu of birth certificate. That being said, if you have to provide school certificates, then you should provide a cover letter stating that there is a middle name that she no longer uses.

This shouldn't be an issue. My wife's surname has changed to my surname after our marriage. I just wrote a covering letter explaining it. You should follow her passport name strictly because that's her official travel document. If you are submitting her Date of birth certificate, both her DOB and Passport names (First and Last names) should match. This means missing of LMN is OK but ABC, XYZ should match. Rest of the differences that you see in any other document are just fine.

If you go strictly with the check list for PR under CEC, not all of your educational documents are mandatory except the certificate of your major education. I have not submitted my school, +2, bachelors except my higher educational certificate. Same is the case with my wife. Best of luck. Write a simple covering letter just as an additional document to make you feel better.

I'm providing an affidavit & 10th class marksheet, as her birth certificate is not available. I'm however planning to apply for her birth certificate in case CIC asks for it later.
