What are the difference between adding a spouse to a PR application and Sponsoring a spouse?

They're completely different things. That's like asking what's the difference between a cheesecake and a banana. You can eat them both for dessert, but they have nearly nothing in common so it's hard to know where you want to start with the differences.
Put broadly, adding a spouse to a PR application means you're a non-Canadian who's applying to become a PR of Canada, and you have a spouse who can get PR at the same time. Sponsoring a spouse means you're a Canadian citizen or PR who wants to bring their foreign spouse into Canada.
It depends on the family member, and on whether all the forms are accurately completed at the time.
For example, some people want to sponsor a cousin - that's quite unlikely to ever be an option. Some people might want to sponsor their parents - for which they'd need to wait years, have proof of income, and get lucky in the sponsorship lottery. Some people might want to sponsor their spouse - so the spouse would need to be properly declared as non-accompanying during the original application, and if they had not been then they would be permanently excluded from family class sponsorship. Etc etc etc.

John is a Canadian PR holder. He got his PR status through his wife who had a PR application in process back in 2013.
The question is:
If John and his wife divorce,how long can they both wait before sponsoring new spouses or common law.
Does the 5 year and 3 years apply to this types of applications?
