We applied for my MIL's vistor visa record in April but still havnt heard anything from immigration yet. she is going back to home country in two weeks now. not sure what to do now in this situation.

Advise IRCC when she leaves. Complete the medical if requested since she benefited from extension of her visit.
 
SHE IS LEAVING NEXT WEEK. I WILL NOTIFY IRCC TODAY. WE HAD EXTENDED THE SUPER VISA MEDICAL INSURANCE SINCE SHE STAYED A BIT LONGER. DO I NEED TO SEND A COPY OF THE INSURANCE TOO?

This has left me a bit confused, as they haven’t yet completed the medical exam, and the visa is valid for one year. Do I still need to arrange for them to complete the medical exam?

The extension approval assumes they will get the medical done. Make sure they do the medical now. We have seen a number of people on this forum in your situation who have ignored the medical and this has created problems later. Again, make sure they do the medical.

But my parents will leave Canada in 5 months which is less than 6 months. Should I call IRCC officer?

No, don't call. Just get the medical done or IRCC will tell you they were non compliant the next time they apply for something.
