If you don't know it..leave they space blank. CIC will trace it (if you have one) and reconcile with your current application. That is why you can still validate the form without UCI# ( you couldn't without vital infos like DOB, Passport # etc).

Yes, you should use the UCI# from visitor visa. CIC does not issue new UCI# for same person. I had the same situation.
