Yeah... you have to go in person to the VFS office (not sure what you'd need to do if its not close to you, may be go to the Indian Embassy with the same documents) with all the documents in the checklist (I believe it is the application, 2 passport size photographs (not the same as Canadian Passport size) and your original passport/ passports). They will take all of it along with the passport and return it to you with the PCC in 2-3weeks.
 
Its been 2 weeks since I got my AOR (or LOA, whatever), and still haven't gotten the medical requests.
Has it taken so long for anyone else?

Same situation here sgblues. I received my AOR on Oct 18th but haven't received my MR yet.

Yea, sometimes MR request are determine by when your application reached your selected VO. Sometimes ago MR were ususally sent with AOR then afther that they start sending MR after about 4-5 days of receving AOR. All this senerios are basically for Ottawa applicant not sure of other VO's.

I got the AoR almost three weeks ago and up to this moment I have not gotten the MR, is it normal? I have also another question, it is mentioed at the AoR email that my case would be soon transfered to a visa office, how long will it take to be received by the VO? will the VO notify me when they get the application? any other feedback that helps me?

When you application get to the VO you selected that when they will send the MR to you. I guess MR has been reduce drastically, because even applicant that choose Ottawa has there VO hasn't received MR yet.. Am sure you application is already with your selected VO
