My view for fast processing:
1. Single applicant
2. Never lived in other countries for more than 6 mos. Hence, only NBI clearance is needed, one country security check.
3. No problem with medical result
4. Visa Officer's performance
5. God's plan

My views to add:

Note some of those who got "faster" processing did come from Backlog Pilot programs. We should factor this in.

Another will be having been into the "system" previously - i.e. history of travel to other countries or resided/worked in the country - thus pre-assessment of the applicant has been started. If you only have one criminality check (NBI) - then only one base from which to make an assessment - you leave the criminality/security assessment to the VO and it becomes a 50-50 chance with the determination of the VO to rely on the country's PCC.

If you have NBI + RCMP records (no PCCs from other countries) and both of them are clear/no record - then you raise the odds of security/criminality check to positively in your favor (makes the decision easier). Plus - a record of having previously assimilated into the Canadian society bodes well for suitability.

And another - political mood in the country. During election times, immigration comes up as a topic; politicians vie for votes from large immigrant populations. You may have noticed that when this government came into power, there was a large wave of approvals for medical professionals in Manitoba and Alberta a few years back. Election time is coming again folks...
