Do I just fill the details of the new passport in the updated information chart and sent the copies of the same? or do i first have to update them about the new passports. - You need to submit details of current valid passports on which visa will be stamped ,
An early response will be really helpful as they haven't given me much time to get back.

1- immigrant visa once issued ; its valid for how many months? - 12 months from the date of Medicals
2- if one is already in Canada after clearing the medical on visit visa .can he/she still can get immigrant visa there while in Canada ? - I am not sure , but one has to send passports to his assigned Visa office , how ever if you are in Canada , and need to send these to London , I believe this will not be an issues , but one has to exit from Canada , and enter on his Immigrant Visa and get COPR stamped
3- if medical is cleared but spouse goes out to country of work can he/she give his passport to stamped for immigr-visa stamped at country of work? - LVO has accommodated many on this , request them when time comes , they will accommodate you on this
