I applied back in March 2012 and submitted full docs in July 2010. Got my 6-12 months wait letter on 20th April.
Today i received a letter from CHC London asking for
1) New Application for Permenant residence IM0008
2) New Schedule A/ Background declaration
3) New Additional family Information Form IMM5406
4) Right of Permenant Residence Fee (Isnt it something that is paid after medicals). Again i am hoping what the are doing????
5) Original Police Certificates for myself and my wife for (Pakistan and Angola) though my wife never stayed there and i too went for a training course for 15 days (Don't know why they are asking for this). I informed them when i moved to UK one and half years ago, but they did not ask one for UK. Strange. Hopefully they know what they are doing?????? Though i doubt
6) Teo fotographs for each family member
7) IMM5562E (Travel Form)
I think to get the forms and refill these is not gonna take more than one week but to get police certificates is gonna be hell of a job as i am here in UK.
Any ideas how i could get this Police certificate from Pakistan in couple of days and could my Abba g apply for this on my behlaf.
What to tell them about Angola?
Should i apply for the UK police certificate now and send it as well as (like i said they are not aware what they are doing).
Is there any website where i could get all these forms all together in one go (as its been a long time i gave up on Canadians and forgot almost all the links). ANY SUGGESTIONS would be appreciated
The BAD THING is they have crossed 90 days and have asked me to produce these things in 30 days. Like i said forms ok but police certificates (Dont the have an embassy in Pakistan, DOnt they know how long it takes to get one).
Anyways, my advice stop wasting time and all of you should start filling the forms (as Like Death and Customers, Canadian Embassy can call you anytime).

Thank you Cuccoo0. For some strange reasons, I was not able to access this forum since last night till now, though all other websites were accessible normally. So, sorry for the late response. I collected the physical copies of the documents CIC had sent from my agent yesterday. I am just listing down what they had requested below:
1) New application for Permanent Residence IMM0008 form for the principal applicant
2) New Schedule A Background/Declaration form for yourself and each family member over the age of 18
3) New Additional Family Information Form IMM5406 for yourself ad each family member over the age of 18
4)Medical Examination
5)Right of Permanent Residence Fee.Visit the Visas and Immigration section of www.UnitedKingdom.gc.ca and click on Fees and Payment Options.
6)Updated copies of the bio-page of the passport for child
7)Updated Original Police Certificates for you and each of your dependents that is 18 years old or more from each country that you or your dependents have lived in for 6 months or more. If you already submitted police certificates for the countries that you and your dependents have lived in since the date you submitted your application. We require the following : Principal Applicant - Sultanate of Oman , Spouse/Common-Law Partner - UAE
8) 2 recent photographs of everyone
9) Other: IMM5562E
These infos were tick marked from a generic form of A4 size
Along with this paper were attached another A4 printout containing 'Instructions for arranging medical examinations'
And most important, a medical report sheet for each of my family member with their photo. (My agent told me some horror stories of mixing up of photos for some other clients where male becomes female and kids photo get interchanged, etc etc!!! - not always though)
Hope this information was useful for those are about to receive their medicals - on what to expect in a normal scenario.
I told Arsh and also told U, about 5 months ago...that your full docs date year is incorrect
