There are documents
1. Job duties for old company. I am getting that letter from my manager(not on company letter head).
2. Birth Certificate. My name is not mentioned on birth certificate. Do i need any affidavit with it or notarized ?
3. In some documents my mother's surname is present in some its not there. Will It create any problem?

thanks for replying. I can't see a place to upload it. Add to that I already have notification from them.
